\u201cRCTs are the gold standard.\u201d
 That\u2019s the mantra we all know in clinical research, and it\u2019s true.
 Or rather, almost<\/em> true.<\/p>

Because when it comes to medical devices<\/strong>, that model starts to crack.
Devices live in a reality that\u2019s dynamic, operator-dependent, and constantly evolving.
Recognizing when an RCT isn\u2019t the right choice doesn\u2019t mean lowering the bar: it means raising the bar for scientific relevance.<\/em><\/p>

Why RCTs don\u2019t always fit<\/strong><\/h2>

RCTs work beautifully for drugs: one molecule, one placebo, two well-controlled groups.
But devices aren\u2019t pills: they\u2019re tools that interact with people, techniques, and systems. And that makes all the difference.<\/p>

Here\u2019s why the \u201cgold standard\u201d can fall short:<\/p>

  1. Learning curve: outcomes depend on user experience. A new robotic tool performs differently as expertise grows.<\/li>\n\n
  2. Rapid innovation: by the time an RCT ends, the device might already have a new version.<\/li>\n\n
  3. Small or niche populations: randomization isn\u2019t always feasible or ethical.<\/li>\n\n
  4. Blinding limitations: you can\u2019t \u201cmask\u201d a surgical tool or software interface; bias will always exist.<\/li><\/ol>

    So instead of asking \u201cCan we randomize?\u201d<\/em> we should ask \u201cIs this the best way to answer the question?\u201d<\/em><\/p>

    The \u201cfit for purpose\u201d approach<\/strong><\/h2>

    Both FDA<\/strong> and European regulators<\/strong> now recognize that what matters isn\u2019t symmetry: it\u2019s scientific coherence<\/strong>.
    The right question isn\u2019t \u201cIs this an RCT?\u201d but \u201cIs this design fit for purpose?\u201d<\/p>
